National collaborations
The High Value 健康care Collaborative
2010年12月, we, along with Mayo Clinic, 丹佛健康, Intermountain 健康care, and The Dartmouth Institute for 健康 Policy and Clinical Practice (TDI), announced the formation of the High Value 健康care Collaborative (HVHC) to improve health care, 更低的成本, and move best practices out to the national provider community. The collaborative identified 9 high-volume, 高成本的, high-variation conditions to focus on, with the goal of improving care and outcomes, reducing variation, and lowering costs. Since its founding, HVHC has expanded to nearly 20 health systems. And its first research, on total knee replacement, has been published.

We remain a member of OneCare because we believe in offering the best care possible to our patients, and because we support the shift to value-based care.
OneCare doesn’t provide health care—instead, it brings health groups together to improve health care quality and stabilize costs. By managing network performance, giving health care providers data and analytics, and paying for quality of care and improved population health, OneCare supports organizations like ours in our work to deliver excellent health care. Through this partnership, OneCare moves 佛蒙特州 toward a trusted, equitable health care system where 佛蒙特州ers and providers work together to achieve optimal health and an exceptional care experience.

The Center for Rural Emergency 服务 and Trauma
The Center for Rural Emergency 服务 and Trauma (CREST) is a Dartmouth Hitchcock Medical Center-based collaborative program of 17 critical access hospitals and community hospitals. Founded at Dartmouth Hitchcock Medical Center in 2007, CREST is designed to improve the emergency care of patients in rural New England and is one of 连关心的 primary educational and outreach partners. CREST is located in Lebanon, 新汉普郡, within our Emergency Department and the Trauma Center, a一级, university-affiliated trauma center.

New England Alliance for 健康
New England Alliance for 健康 (NEAH) is a group of community hospitals, behavioral health centers, and home healthcare agencies that came together in a shared commitment to improve the quality, 效率, and availability of health care in 新汉普郡, 佛蒙特州, and western Massachusetts. As a group, NEAH members share knowledge, resources, and solutions to challenging healthcare issues. In our local communities, we treat illness and improve wellness. We work with tax-exempt charitable health care organizations in thoughtful, patient-focused collaboration to improve patient care systems and the health status of all communities in the region.
